Appendix D — Notes
- Note 1: This form should be completed for every dangerous goods occurrence, whether or not a formal accident/incident has occurred.
- Note 2: The report should be submitted as soon as possible, but no later than 72 hours after the occurrence.
- Note 3: If multiple UN numbers are involved, complete a separate dangerous goods information section for each substance.
- Note 4: Photographs of damaged packages, labels, markings, and the scene should be attached whenever possible.
- Note 5: If the occurrence involves undeclared dangerous goods, describe how the goods were identified and any visible markings.
- Note 6: The competent authority of the State of occurrence AND the State of the operator must both receive a copy.
- Note 7: Retain a copy of the completed report, DGD, AWB, and any supporting documents for a minimum of 3 months.
- Note 8: For occurrences involving radioactive materials (Class 7), additionally notify the radiation protection authority.
- Note 9: This form may be adapted by individual operators, but must contain at minimum the fields specified in DGR Appendix D.
